| Description: | Berks Visiting Nurse Association is currently seeking a full time Team Manager to coordinate, implement, and evaluate the delivery of clinical services under the direction of a Clinical Manager – Primary Nursing. The Team Manager is responsible to supervise assigned personnel, and oversee case management of all home health clinicians including daily functions in providing patient care. Other responsibilities include ensuring coordination and implementation of established client care plans, and efficient management of the multidisciplinary team, and related activities. The Team Leader assists in the maintenance of appropriate staffing, monitors service utilization for appropriateness, provides guidance to staff related to appropriate visit frequency and duration, oversees scheduling activities for all clients on assigned team, and assists in the monitoring of staffing patterns. The Team Leader is also responsible to assist with recruitment, interviewing, selection, evaluation, discipline and discharge of clinical staff; assist in the implementation of the staff orientation; act as a resource to staff in matters of policies and procedures; participate in client care conferences and in-service programs; supervise the timeliness and quality of documentation of assigned staff; assist in the evaluation of the performance of assigned staff in meeting productivity and qualitative standards, and support customer service programs by promoting and maintaining a responsive knowledge and accessible approach to all customer contacts. REQUIREMENTS Graduation from a State accredited Nursing Program and valid professional RN license to practice in the State of Pennsylvania, 2 to 3 years home health nursing experience, minimum one year management experience, and strong leadership skills. Strong computer skills, familiar with computerized documentation, comprehensive knowledge of professional nursing theory and the ability to apply it to the community health setting; Ability to communicate effectively, both orally and in writing; ability to exercise leadership coupled with tolerance, understanding, appreciation and respect for the special abilities and skills of others; and ability to maintain client confidentiality. Valid Pennsylvania Driver’s license, automobile insurance, available automobile during working hours and able to drive in all types of weather. . |
