Telehealth
Telehealth is a monitoring system to help manage and educate our clients and families on disease and symptoms by using standard phone line and electrical outlet using video and non-video monitoring systems.

Additional disease management programs:

  • Congestive Heart Failure (CHF)
  • Diabetes
  • Hypertension

Types of clients who are eligible for Telehealth services:

  • Clients with respiratory problems, congestive heart failure, diabetes, or hypertension.

Outcomes and Benefits of utilizing the Teleheath program:

  • Educate our clients on their disease and symptoms and to prevent unplanned doctors visits, trip to the emergency room, or readmissions to hospital due to unmanaged symptoms.

Our employees need to be a licensed nurse and trained to use the equipment in order to troubleshoot problems and educate clients on how to use the equipment.
